Of all the misalignments of the first ray of the foot, dorsal bunion is the least well known. It is defined by a reduction or absence of ground contact of the first metatarsal head and flexion of the hallux metatarsophalangeal joint. Our aim is to describe a surgical technique inspired by the LAPIDUS technique for the correction of dorsal bunion. 

Case history:
A patient, 30-year-old, with a dorsal bunion of the first ray of iatrogenic origin (a posteromedial release performed at the age of 18 months for congenital clubfoot varus equinus (CBVE)). He underwent an operation combining a dorsal opening osteotomy of the Cuneo-metatarsus of the 1st ray, incarceration of a triangular bone graft with a dorsal base at this level and fixation with a staple, disinsertion of the distal tendon of the hallux longus flexor and its transfer to the neck of M1 by introducing it into a tunnel dug at this level. The patient was stabilized with a plaster cast maintained for two months. Postoperative recovery was straightforward, and the functional result was satisfactory.

Discussion and conclusion:
Dorsal bunion is a rare and poorly understood deformity. It causes both functional and aesthetic discomfort. It was Lapidus who coined the now classic term "dorsal bunion" and developed an operating technique that bears his name. Several procedures have been proposed in the literature, the majority of which are based on Lapidus' procedure.
