These are complex injuries that usually occur during high-velocity trauma. If left untreated, they result in significant loss of function, pain and long-term instability. In the absence of a consensus in the literature on optimal treatment guidelines, single-stage or two-stage operative management remains a matter of debate. Case presentation: A total of seven patients with multi-ligament knee injuries were managed between 2018 and 2021 at the Kairouan Orthopedic Department.All of these patients had single-stage reconstruction. Outcomes were evaluated by Lysholm, and IKDC scores with a minimum 14-month follow-up. ACL injury was found in 100% of cases. It was accompanied by a PCL rupture in 03 cases, and a posterolateral corner in 02 cases. The association of ACL, PCL and posterolateral coroner was noted in one case. The seventh case involved an association of PCL and with medial collateral ligament , No patient had neurovascular lesions. Discussion : Multi-ligament injuries of the knee are a rare entity, They cause a significant functional handicap. Adults who undergo anatomic reconstruction of multi-ligament knee injuries have excellent postoperative results.The sequence of graft tensioning and fixation in one-stage surgery remains controversial. Autograft options for ligament reconstruction are limited. Some authors have achieved better results with one-stage reconstruction with results similar to our study. Nevertheless, the longer operative time could be a source of complications such as infection, stiffness and instability, On the other hand, a multistage repair could improve clinical results by decreasing the rate of stiffness and thus possible unplanned revision surgery. However, it prolongs the duration of immobilization and delays rehabilitation.
