Clinical examination is crucial for guyon’s canal syndrome diagnosis , however it can be misleading due to the variations of clinical presentations especially when the sensory symptoms are absent or atypical. We report a case of a 20 year’s old male admitted in our departement for a guyon’s canal syndrome due to an aneurysm of the ulnar artery and had a surgical decompreesion. The decompression surgery was carried out under general anesthesia and The aneurysm was carefully removed and the ulnar artery was ligaturated after a favorable allen test. After three months the patient was pain free , progressively gaining his hand function and very satisfied with the proceduremany etiologies of Guyon's canal syndrome were reported and can be classified into groups. Treatment options depend on severity and duration of symptoms, previous treatment attempts, and underlying etiology. Adjacent vascular enlargement is not a common cause of Guyon's canal with few case reports in the literature.
