Besides immediate complications, the prognosis of these injuries is burdened with osteonecrosis, arthritis, and nonunion. The aim of this study is to evaluate the short term functional and radiological outcomes of surgical and orthopedic treatment Methods : This is a retrospective study of 26 active adult patients over 17 years treated for fractures and/or dislocations of the talus using orthopedic or surgical methods. Functional evaluation was performed using the HAWKINS score. Radiological assessment at final follow-up searched for necrosis, arthritis, and nonunion. Results: The results were evaluated with an average follow-up of 12 months. according to HAWKINS score results were excellent in 46% of cases, good in 35% of cases, and fair in 19% of cases. Anatomically union was achieved in 69% of cases. Nonunion was present in 19% of cases, malunion in 12% of cases, arthritis in 35% of cases, necrosis in 27% of cases, and anterior impingement in 8% of cases. Post-treatment pain was influenced by capsuloligamentous injuries and the type of surgical treatment. Joint stiffness was more common in patients who underwent surgery and those with ligamentous injuries. Functional outcomes varied depending on the type of injury and treatment, generally yielding good results for subtalar dislocations and partial fractures. However, talar extrusions were associated with less favorable outcomes. Anatomically, bony union was better in the absence of ligamentous injuries, while surgical treatment was associated with an increased risk of complications such as osteonecrosis and arthritis, especially in open fractures. Osteonecrosis was influenced by the nature of the injuries and was more common in surgically treated patients.
