This ailment typically affects the second metatarsal head, although other metatarsals can also be involved. The manifestation of Freiberg's disease is often insidious, with symptoms progressing gradually over time. Objectives: The primary objective of our study is to systematically investigate and present the efficacy of dorsal flexion osteotomy as a therapeutic intervention for Freiberg's disease, with a specific focus on elucidating the favorable functional outcomes observed. Methods: A retrospective analysis was conducted to assess the outcomes following dorsiflexion osteotomy in 10 patients. Patient contentment, clinical observations, and radiographic measurements were meticulously examined, with an average follow-up period of 24.7 months. Dorsiflexion osteotomy was carried out on 8 women and two men who were experiencing symptomatic Freiberg's disease and had not responded to conservative treatments. Their average age was 29 years. Among the cases, nine involved the second metatarsal, while one case of the third metatarsal. The disease's progression was classified using Smillie's classification. Of the patients, seven were at stage V, two at stage IV, and one at stage II. The surgical technique entailed a wide metatarsophalangeal arthrotomy to expose the metatarsal head. The osteotomy was stabilized using an absorbable suture, and the foot was placed in a plaster shoe cast for a period of three to four weeks. Results: The procedure yielded outstanding results for all patients, demonstrating minimal reduction in metatarsophalangeal mobility and an average reduction of 2.3 mm in metatarsal length based on X-ray assessments. Moreover, postoperative metatarsalgia was absent. No instances of infection, avascular necrosis, or pseudarthrosis were observed. Complete pain relief was achieved, enabling all patients to engage in sports and activities that were not feasible prior to the surgery. Metatarsophalangeal flexion saw a decrease of 15°, while metatarsophalangeal extension experienced a decrease of 10°, without causing any disruption in gait.
