The medial ligaments of the thumb's metacarpophalangeal articulation are comparable to the cruciate ligaments of the knee. If left untreated, there is a risk of chronic laxity, leading to instability and eventually to painful degenerative osteoarthritis, which can often become disabling.
Methods:
This is a retrospective descriptive study conducted over a 3-year period, involving 10 patients who underwent surgery for a recent severe sprain of the medial collateral ligament (MCL) of the thumb metacarpophalangeal articulation. This study was carried out with a minimum follow-up of 1 year, and was performed at the Orthopedic and Trauma Surgery Department of the Internal Security Forces Hospital in La Marsa.
Results:
This study of 20 patients who underwent surgery for severe sprain of the medial collateral ligament (MCL) of the thumb metacarpophalangeal joint revealed the following results: Patients had an average age of 35 years and 6 months, and men predominated. Sports- and entertainment-related accidents were the most common cause, occurring in 60% of patients, with an average delay of 17 days before medical management. All patients presented with pain and hyper-laxity. In 26% of cases, the severe LLI sprain was associated with a bone fracture, mainly a small bone pad. Surgical procedures were performed under locoregional anesthesia, using a variety of surgical techniques. Therapeutic results were assessed in terms of absence of pain, joint stability, thumb opposition ability and patient satisfaction, showing a high rate of good results, with 75% very good and 19% good.
Conclusion:
Metacarpophalangeal sprains of the thumb are common, but their gravity is often underestimated. A precise clinical examination of the thumb, supplemented by static and dynamic X-rays, as well as the use of ultrasound, are necessary to exclude a severe sprain. If this is not the case, surgery should be considered to prevent a possible progression to instability.
