            [0] => Adolescent idiopathic scoliosis is surgery reputed with good prognosis. But complication such as infection, neurologic and  posterior non fusion can worst prognosis and  complicate the management.  
We collect patients from 2012 until 2021. Inclusion criteria were age <16 years, AIS, and minimum 2-year follow-up. Date and type of complication were collected and classified into three severity groups (minor, major, major leading to a reop). Only complications occurring before the 2-year visit were retained for analysis. 
Results 
657 patients were analysed. The mean age at the time of surgery was 14,7yo±2.3, 81.3% were female, and 13,5% had a history of previous spine surgery. Surgical data were typical of AIS  surgery (95,7% posterior fusion, 15,7% osteotomy). 
The overall complication rate was 8,11% (N=81). 12 patients (1,8%) experienced at least one complication the day of surgery (including intra-op), 29 (4,4%) between POD1 and 6-weeks post-op, 21 (3,1%) between 6 weeks and 1-year post-op, and finally 20 (3%) between 1-year and 2-years post-op. Stratification by type of complication demonstrated that most of the medical complications occurred within the first 2 months. Surgical complications : operative, wound, and infection occurred early (within 60 days), while implant-related and radiographic complications happened at a constant rate over the 2-year period. Neurologic complications had a high occurrence with the first 60 days, followed by a continuous increase up to the 2-year visit. 
Discussion:  10 percent of  AIS patients remained complication-free by 2 years, and 1  patient  out of hundred will have had a complication requiring a reoperation/revision. Estimation of timing and type of complication associated with surgical treatment is crucial when counseling patients and planning treatment cost-effectiveness
Conclusion: Complication in AIS   is surgery as infection, neurologic and  posterior non fusion can worst prognosis and  complicate the management.
