Traumatic anterior dislocation of the hip, though rare compared to posterior dislocations, presents a significant orthopedic challenge, accounting for 5% - 18% of all hip dislocations. It is often the result of high-velocity accidents, such as deceleration incidents or motorcycle collisions, causing the limb to be in a position of abduction, external rotation, and flexion. This report focuses on obturator dislocation cases, a subset of anterior hip dislocations, discussing treatment and prognosis.
Observations:
1.	Case 1: Mr. BA, involved in a road traffic accident, presented with right obturator dislocation and a left supracondylar open fracture. Closed reduction stabilized the hip, and surgical treatment addressed the fracture, leading to satisfactory outcomes.
2.	Case 2: Mr. CA experienced a right hip injury from a collision, resulting in obturator dislocation. Closed reduction and traction treatment yielded positive results without signs of avascular necrosis.
3.	Case 3: SA suffered multiple injuries from a motorcycle accident, including left subtrochanteric fracture and right obturator dislocation with an incarcerated fragment. Despite attempted reduction and surgical intervention, subsequent complications like early coxarthrosis emerged.
4.	Case 4: Mr. DS presented with left hip obturator dislocation following a motorcycle accident. Closed reduction and traction treatment prevented avascular necrosis, as confirmed by follow-up MRI.
Discussion:
Anterior hip dislocations, including obturator dislocations, typically result from high-energy trauma and require immediate attention due to associated injuries. Prompt reduction is crucial for functional prognosis, ideally within 6 hours post-trauma. Diagnosis relies on clinical and radiological assessments, with standard radiographs confirming the dislocation. Surgical reduction may be necessary in some cases, especially with associated fractures or intra-articular fragments.
Complications such as avascular necrosis and coxarthrosis are significant concerns, influenced by the timing of reduction and treatment of associated injuries. Follow-up assessments, including bone scintigraphy or MRI, are essential for monitoring femoral head necrosis.
Conclusion:
Obturator dislocation of the hip, although uncommon, necessitates urgent recognition and intervention. Treatment primarily involves closed reduction, but vigilance for associated injuries and complications like avascular necrosis is crucial. Regular follow-up assessments are vital for long-term management and monitoring of femoral head health.
