The challenge lies in achieving bone union in this area subjected to significant mechanical stresses. The Matti-Russe technique is effective in treating scaphoid non union, offering an intriguing approach to addressing this challenge. The aim of this work is to present a novel approach inspired by this technique for addressing non union after olecranon osteotomy. Methods: We present a technique for treating non union following an olecranon osteotomy in a 32-year-old female patient with history of rheumatoid arthritis, inspired by the Matti-Russe method, combining bone grafting and osteosynthesis. The osteotomy was carried out during a surgery for a supracondylar and intercondylar fracture of the right elbow. Result: At three months, radiographic assessment confirmed bone heeling at the non union site. At six months, significant improvement in elbow mobility was observed, allowing the patient to resume daily activities. After one year, the osteosynthesis material was successfully removed. Discussion and conclusion: Articular olecranon osteotomy provides good exposure of distal humeral fracture pattern but is associated with non union risk. Our choice of surgical method draws from the Matti-Russe technique renowned for its effective bone healing.
