Tuberculosis is a major public health problem worldwide. Osteoarticular tuberculosis is the fourth extra-pulmonary localization. Its diagnosis does not pose a problem in cases of concomitant bacilliform pulmonary localization, while it is often difficult and late in cases of isolated joint involvement .
Patient and observation:
We report an observation of isolated tuberculous arthritis of the knee in a 19 year old adolescent whose diagnosis was late and which was made by the detection of the genome of the tuberculosis complex. The patient was placed on medical treatment under the 2RHZE/7RH protocol (Rifampicin, Isoniazid, Pyrazinamide, Ethambutol) for nine months with rehabilitation and early mobilization of the joint. The development was favorable. 
Discussion :
 Tuberculosis is the leading cause of death linked to a curable infectious disease. Due to the insidious and non-specific clinical presentation, the diagnostic delay is often long with major joint functional impact.
 The diagnosis of tuberculosis remains bacteriological despite the difficulty of isolating the pathogenic agent in extra-pulmonary forms qualified as pauci-bacillary, particularly osteoarticular involvement.
Conclusion
Through our observation, we emphasize the interest of sampling for bacteriological purposes in the face of any chronic arthritis, with the search for Mycobacterium tuberculosis preferably using new molecular biology methods which have a reduced response time compared to culture.
