Isolated posterior radial head dislocation is an extremely rare injury in adults. While it is frequently described in children, in aduIts it is usually associated with ulnar fractures. The diagnosis may be easily missed on radiographs. 
Case presentation: 
We report the case of a 20-year-old man who consulted the emergency department following a direct impact by a chair on the radial side of the forarm. He presented with a restricted range of motion of the right upper limb, swelling and pain of the elbow. On examination the elbow was held in flexion and partial supination. The radial head protruded posteriorly. Forearm and distal radioulnar joint were painless.
Neurovascular examination did not reveal any abnormalities. 
X-rays showed a posterior dislocation of the radial head with normal humeroulnar congruence. There was no evidence of fractures or other concomitant injuries, neither lesions on homolateral shoulder or wrist.
Close reduction was performed under general anesthesia, light traction, pronation and posterior pressure were applied to the radial head. The elbow was stable from 0° to 130° of flexion and in complete pronosupination. Immobilisation in a long arm cast was carried out for 3 weeks in neutral rotation. Further screening was done weekly to make sure that the radial head was in place.
Clinical outcome:
Active elbow mobilization was started after removal of the cast and the patient underwent a supervised physiotherapy program. 
At 3 months, the patient had recovered clinically and functionally, with complete range of motion.
Discussion and conclusion:
Isolated dislocation of the radial head in adults without concomitant ulnar fracture is a rare injury. Most cases occur in children. Only 23 cases have been reported in adults in the literature over a period of 45 years with predominantly posterior cases.
Most authors have described an indirect mechanism, but in our case the patient received a direct shock to the forearm in pronation and hyper-extension.
There are no guidelines for treatment. In the literature, 15 cases were treated conservatively with no recurrence. This is an additional case treated conservatively with a good outcome.
