Isolated anterior dislocation of the radial head is exceptional. When it is old, it poses a problem of differential diagnosis with congenital dislocations.
CASE PRESENTATION
A five-year-old right-handed child with a traumatic injury to the right elbow, characterized by an isolated anterior dislocation of the radial head, was successfully treated by urgent reduction under anesthesia. Following four weeks of immobilization, the child exhibited normal elbow function and radiographs confirmed the proper alignment of the radial head. A one-year follow-up showed no abnormalities, indicating a successful recovery without residual issues.
DISCUSSION 
Isolated anterior post-traumatic dislocation of the radial head is a rare condition often associated with Monteggia fracture dislocation. Controversy surrounds its existence, with some considering it equivalent to Monteggia fracture dislocation. Mechanisms of injury vary, including forced supination or hyperextension. Anatomically, ligament damage is crucial for dislocation occurrence. The clinical examination is nonspecific, necessitating X-rays for diagnosis. Treatment involves closed reduction under anesthesia and immobilization. Failure in recent dislocations may require open reduction. Chronic dislocations, present beyond three weeks, necessitate surgical intervention. Postoperative outcomes depend on the dislocation's age and the surgical approach.
CONCLUSION
Isolated anterior traumatic dislocation of the radial head must be considered in the face of any acute trauma to the elbow. Radiographic images allow the diagnosis and the treatment is orthopedic for acute forms, while in neglected forms surgery is the treatment of choice.
