Introduction: Intra-articular osteoid osteoma is a rare benign bone tumor that can present with atypical symptoms, mimicking arthritis and posing a diagnostic challenge for clinicians. This study reports a case of intra-articular osteoid osteoma of the knee and describes the clinical, radiological, and therapeutic aspects of this condition.

Case Presentation: A 14-year-old child presented with acute left knee pain and fever. Initial investigations, including radiographs and arthroscopic joint wash, were inconclusive. Persistent pain and limping prompted further imaging with MRI and CT, revealing a nodular lesion suggestive of osteoid osteoma. The diagnosis was confirmed by bone scintigraphy and pathological examination after CT-guided drilling resection. The patient experienced complete resolution of symptoms post-operatively.

Discussion: Intra-articular osteoid osteoma frequently presents with misleading symptoms, leading to delayed diagnosis and management. The classic radiographic features of nidus and peripheral osteosclerosis are often absent, necessitating the use of multiple imaging modalities. Minimally invasive percutaneous techniques have become the treatment of choice, with high success rates. However, intra-articular location requires careful approach to prevent joint damage.

Conclusion: Intra-articular osteoid osteoma of the knee is a challenging diagnosis due to its atypical presentation. A combination of clinical assessment and various imaging methods is often necessary for accurate diagnosis. Percutaneous surgery is the gold standard treatment, with special considerations for intra-articular lesions to minimize cartilage damage.
