Femoral neck fractures are rare in young adults. With the emergence of arthroplasty, conservative treatment for patients aged 45-55 becomes heavy with reduction difficulties, delay in weight bearing and risk of implant failure. 
Methods
We conducted a retrospective and descriptive study in Charles Nicolle’s Hospital over a period of ten years. We included all patients aged between 16-55 years old, who were admitted for GARDEN III and IV femoral neck fractures. Osteoporotic and pathological fractures were excluded. We evaluated age, smoking and cause of trauma, radiographic findings, treatment, complications and functional results using Friedman and Wyman classification.

Results
A total of 30 patients were included. Mean age was 46. Four patients were smokers. Predominant mechanism was domestic fall. Comorbidities were evaluated based on ASA classification and was mainly ASA 1 in 70%. All Femoral neck fractures were classified GARDEN IV. Patients had surgery within a mean period of 6 days (1-30). 
All fractures were operated on : conservative treatment using close reduction + DHS 2 Holes with a cancellous screw (37%), cancellous screws (3%), Open reduction using DHS 2 Holes with a cancellous screw (3%) and arthroplasty using a THA (57%). Weight bearing was authorized within 90 days in conservative treatment.
In conservative treatment, postoperative AP pelvic Xray showed good reduction in 10 cases, varus deformity in three cases, correct implant placement in 10 and fracture shortening in 1 case. Three cases of nonunion were noted where THA was performed within one year. One case of aseptic loosening of THA resulted in revision which was complicated with periprosthetic joint infection. Over 50 years, we performed 68% THA over 32% of conservative treatment. Functional results according to Friedman and Wyman classification were good in 20 cases of which 19 THA, fair in 7 cases with moderate pain and poor in 3 cases. 

Conclusion
Conservative treatment of femoral neck fractures in young adults is a cost-effective solution preserving the hip. However it has a high complication rate and may end up in performing THA. Primary THA is interesting. The choice remains incertain for patients aged 45-55 years old.
