Methods We reviewed 43 deformity patients from 2015-2018. We derived radiographic metrics describing coronal alignment: the horizontal distance between the C7 plumb line and the CSVL (C7-CSVL). We divided patients into coronally aligned (CA) and malaligned (CM) groups based on CM as C7-CSVL>4cm in 2yr follow up. Logistic regression and Chi-square analyses evaluated the relationship between the postop C7-CSVL with SRS (difference between pre and 2 yr postop) scores. Results : Of the 43 patients, there were 7 with CM when it was defined as postop C7-CSVL>4cm. There were statically insignificant and minimal differences between the SRS total scores (20.5 vs. 9.7, p=0.03) in the CA and CM group when it was defined based on CSVL. Discussion: CSVL is a radiographic representation of coronal alignment after spine deformity surgery that directly correlates with SRS scores but statistically non insignificant. Conclusion: There is a paucity of literature demonstrating a reliable relationship between radiographic coronal malalignment CSVL and clinical SRS score. Our study demonstrated the correlation with increase clinical SRS score but not statistically significant. ) [rank_math_internal_links_processed] => Array ( [0] => 1 ) [ple-content_has_sections] => Array ( [0] => ) [_eael_post_view_count] => Array ( [0] => 16 ) [trx_addons_post_views_count] => Array ( [0] => 14 ) [ekit_post_views_count] => Array ( [0] => 14 ) [trx_addons_post_likes_count] => Array ( [0] => 0 ) )
