Debates exist whether this kind of fracture should be treated by conservative or operative method. Closed reduction and casting, is a cheap, non-invasive treatment, which can be done without a hospital stay. However, this method cannot perfectly realign the bone fragments and joint cartilage. Operative management is associated with typical anesthesia and surgery risks, exposure to radiation, and financial expenses. But allows anatomical reduction and early rehabilitation. The aim of this study is to compare the functional and radiological outcomes of conservative and operative management of distal radius fractures. Methods We performed a retrospective, longitudinal, descriptive study of 57 patients, of 60 years-old or more, who presented with a dorsally angluated, extra-articular fracture of the distal radius. 27 patients underwent a closed reduction with long arm cast for 6 weeks, and 30 patients underwent a percutanous pinning. Patients were evaluated at one, two and five weeks, after four months and at one year. Functional outcomes includes ulnar deviation, radius deviation, flexion, extension, grip strength The intensity of pain was recorded on a visual analogue scale from 0 to 10. The primary radiographic outcomes include ulnar variance, radius frontal inclination and radius anteversion. Results There was no statistically significant difference between the two groups in range of motion at one year, except for ulnar deviation (p = 0.009). Pain score was higher in the closed reduction group, but the difference was not significant. There was a significant difference for radial frontal and sagittal inclination but not for ulnar variance at one year. Discussion Soffelen and Broos carried out a prospective, randomized trial that compared closed reduction with intrafocal pinning for extra-articular fractures. Their findings revealed no difference in outcomes between the two groups. This is in accordance with our study. The difference in radiological outcomes did not translate to a difference in functional outcomes.
