Bilateral acetabular fractures are very rare. They are usually seen in the context of multiple trauma patients. This type of fracture poses a problem and difficulty in therapeutic management, sometimes resulting in serious sequelae and significant morbidity.
CASE PRESENTATION
A 47-year-old trader involved in a car collision presented with chest pain and severe hip pain. His previous hip surgery complicated the assessment. Examination revealed rib fractures, hip deformities, and superficial injuries. A total body CT scan showed rib fractures but no thoracic or abdominal injuries. After initial pain management, he was admitted, underwent hip surgery, and was prescribed bed rest and physiotherapy. At the 3-month follow-up, the left hip's non-displaced fracture was treated conservatively, while the right side showed satisfactory postoperative results.
DISCUSSION 
The increasing incidence of acetabular fractures, often linked to high-energy automobile accidents, is rising. Bilateral acetabular fractures are rare, commonly resulting from dashboard injuries in car crashes. Other mechanisms include being trapped between a vehicle and a wall, road accidents, airplane crashes, falls, seizures, sports accidents, and agricultural mishaps. High-energy trauma requires prompt surgery, and proper classification using Judet Letournel's system is crucial. Surgical delay may not significantly affect the reduction degree, but osteoarthritis is a common complication. Heterotopic ossifications can be managed with indomethacin. 
The main goal of surgical treatment is achieving anatomical fracture reduction for the hip's future.
CONCLUSION
The treatment goal of acetabular fracture is anatomic or near anatomic reduction of the articular surface. The risk of osteonecrosis is mostly related to the initial injury and time to reduction
