The fungal origin is exceptional, generally affecting immunocompromised patients. Candida Glabrata is exceptionally involved. Through a case report and literature review we tried to elucidate risk factors of this complication, its treatment and practical recommendations to avoid it. We report a rare case of Candida Glabrata septic arthritis complicating arthroscopic ACL ligamentoplasty in an immunocompetent person. The patient had a surgical washout, a synovectomy and an antifungal treatment. The clinical and biological evolution has been good. The clinical signs are similar to conventional septic arthritis, except that the evolution is much slower in fungal arthritis. However, it can cause serious osteocartillaginous damage. Joint puncture is the key of the diagnosis. We recommend doing mycological research, in addition to bacteriological research, in order to not miss a fungal germ, which is, although rare, can cause serious osteoarticular damage if not treated early and adequately. The treatment is medico-surgical. Therapeutic management is not unanimous. The first objective of the treatment is to protect the articular cartilage and to preserve the graft.
