Bilateral Achilles tendon rupture is rare (2 per 1000). It is often associated with drug-induced tendinopathy or secondary to systemic diseases or chronic renal failure. Using a case of bilateral Achilles tendon rupture, we will attempt to elucidate the etiopathogenesis of this lesion, to clarify its revelatory nature in relation to other underlying pathologies, and to elucidate its therapeutic difficulties. 
CLINICAL CASE 
Mrs. S.Y., 54 years old, hypertensive, consulted us with pain in both ankles and total functional impotence for three weeks, following an effort to lift a heavy load. The patient had never presented with enthesopathy, arthropathy or other systemic pathologies. She had also never been on long-term medication. Clinical examination and standard radiographs showed a bilateral Achilles tendon rupture. The diagnosis was confirmed by ultrasound examination. Intraoperative exploration revealed an irreducible interfragmentary gap. Surgical repair of both tendons was performed using the Bosworth technique. To protect the plasty, immobilization was maintained for three months. Weight-bearing was allowed after 45 days. Rehabilitation began after 3 months. After six months, the patient was walking again, despite reduced joint mobility in both ankles. 
Conclusion:
Bilateral Achilles tendon rupture is a rare injury which presents therapeutic and evolutionary difficulties. The absence of a reference to determine and evaluate the physiological equinus during tendon repair, and the obligation to defer weight-bearing for 45 days, are two factors that place constraints on both the patient and the surgeon. All the more so as this injury may conceal a systemic pathology that needs to be explored and treated.
