Tillaux fracture is an articular fracture of the distal tibial epiphysis in children and adolescents. However, it is exceptional to find such a lesion in adults. Clinical and radiological diagnosis is sometimes difficult, especially in the absence of any displacement, hence the need to complement investigations with a CT scan. The classical treatment is open reduction and screw fixation, with prolonged recovery time.
Objectives
The aim of this case report is to detail the surgical technique and evaluate the functional and radiological outcomes of the arthroscopic approach in the treatment of a tillal fracture in an adult: a very rare situation.
Study Design & Methods
We present a clinical case of a Tillaux fracture in an adult, successfully treated with an arthroscopic approach.
Results
This case study demonstrates the effectiveness of the arthroscopic approach in the management of Tillaux fractures. It enables us to establish a precise lesion status, control reduction directly and restore the articular space anatomically. We can conclude that this technique is superior to the classic approach, while being much less invasive in terms of skin condition, edema and pain after the operation. Post-operative recovery is faster with this technique, allowing earlier mobilization.
Discussion
Although Tillaux fractures in adults are exceptional, and the literature reports only sporadic cases, they must be optimally managed in view of the possible complications. Given the promising results of the arthroscopic approach compared with open surgery, in terms of reduced soft tissue damage, simpler postoperative follow-up and faster functional recovery, this technique could become a preferred treatment option for this type of fracture, and could also be extended to other types of articular fractures.
Conclusions
Arthroscopy is emerging as a promising option for the treatment of Tillaux fractures in adults. However, more extensive clinical experience is needed to support these advantages. This approach remains an exciting avenue for the future of trauma surgery.
