We report an exceptional case of simultaneous fracture of the lateral femoral condyle and the patella. Observation: We are presenting this case report of a 51-year-old male patient with no previous pathological history, who was the victim of car accident causing an open trauma to the right knee. The clinical examination showed total functional impotence, ecchymosis of the anterolateral aspect of the knee with a punctiform external skin opening. X-ray: longitudinal fracture of the patella with longitudinal fracture of the lateral femoral condyle CT scan: slightly displaced comminuted uni-condylar articular fracture of the lower end of the femur with an external sagittal line plus displaced fracture with a vertical line of the patella with an inter-fragmentary gap of 8 mm Was operated on the same day with patella Tension Band Wiring plus external condyle screw fixation using the same approach. The post-operative follow-up and functional outcome were good Discussion: The orthopaedic literature on the management and classification of uni-condylar fractures of the femur is controversial. These fractures are generally neglected because of their frequent association with other lesions ,they involve the lateral femoral condyle three times more than the medial condyle, essentially explained by the physiological valgus. This case of simultaneous homolateral fractures, unicondylar sagittal and patellar vertical is extremely rare. Such a combination of fractures has not yet been reported in the literature. In our case, we thought that the mechanism of these combined fractures results from the transmission of the forces of direct trauma from the patella to the lateral condyle. A CT scan is required to provide a more precision. We used the same antero-external approach for the reduction and synthesis of the two fractures. Conclusion: The combination of patella fracture and fracture of the lateral femoral condyle represents a rare anatomical entity.
