Fractures of the distal tibia square occur on a superficial tibial segment exhibiting at frequent complications. The therapeutic methods are numerous but the lack of consensus makes the choice difficult. 
METHODS
A retrospective study of 35 patients who presented 35 fractures of the distal square of the tibia and who were treated during a period of 6 years fromJanuary 1, 2016 to December 31, 2021.
RESULTS
The average age of our patients was 53 years with extremes ranging from 18 to 82 years. The sex ratio was 3/2 , these fractures were secondary to a direct mechanism in 87.5% of cases.The average processing time was 4 days with a maximum of 26 days. Orthopedic treatment was the most used therapeutic method in our series. A deep infection was observed in 2 Cauchoix 3 open fractures and progressed well under antibiotic therapy and local care.
Malunions were the most common complication
CONCLUSION
Our study discusses classifications for fractures of the distal square of the tibia, emphasizing the drawbacks of orthopedic treatment, including high rates of unfavorable outcomes. Surgical methods, particularly intramedullary nailing, are increasingly favored due to advances in techniques, early support, and lower complication rates. Authors recommend specific criteria for successful nailing, highlighting the importance of intact bone margins. Fixation of the fibula is crucial, as it enhances stability and reduces malunion risks. Open reduction and internal fixation are considered favorable alternatives, providing anatomical reduction and functional recovery. External fixation is reserved for specific cases, with associated complications such as ankle stiffness, malunion, and pin infection.
