These are dominated by avascular necrosis (AVN) of the femoral head, and nonunion. Many parameters were reported in the literature with a direct or indirect effect on failure or success. The aim of our study is to determine the risk factors for failure of conservative treatment of the femoral neck in patients aged less than 65 years. Methods: It is a retrospective comparative study of three series of NOF fractures treated surgically over a period of ten years; including young adult patients treated conservatively. Patient characteristics, time to surgery were collated; surviving patients were followed up at a minimum 24 months. Reduction quality was assessed on X-rays in 3 dimensions Results: Our series included 75 patients. Average follow-up was 5.4 years [2 years-12 years]. At the last follow-up, Fifty-two patients (69.3%) healed without complications. AVN occurred in 14.7% of the patients. Non-union was found in 16% of cases. The age, sex, smoking habits, alcohol use, violence of the trauma and the delay in treatment did not appear to be risk factors for progression to complications. Garden classification was significantly predictor of the development of AVN (OR = 4.5 [95% CI: 1.49-17.62] and p = 0.002). Survival appeared to vary according to the quality of reduction with significant statistical tests (OR=4; 95% CI [1,95 - 16,79] and p=0.046).
