Introduction :
Traumatic anterior dislocations of the hip without fracture of the acetabulum or femoral head are rare. Resulting from high-velocity trauma, these injuries often involve associated fractures. They are often secondary to public road accidents and rarely encountered in sports.
Case presentation :
An 18-year-old roller skater suffered an obturator dislocation of the left hip following a collision with a colleague's bike. Initial examination revealed total functional impotence of the left lower limb. Emergency reduction under general anesthesia was performed successfully, followed by a post-reduction CT scan confirming reduction and absence of associated intra-articular lesions. The patient was discharged after 15 days and advised to avoid external rotation movements for four weeks. Sporting activity resumed without issues after six months.
Discussion :
Traumatic obturator dislocations are rare but often associated with fractures of the acetabulum or femoral head. They typically result from a forceful movement involving flexion, abduction, and external rotation of the hip joint. Anterior capsular lesions are common, potentially leading to irreducibility. Injuries may also include fractures of the femoral head and cartilage lesions. Reduction can be challenging and may require general anesthesia to prevent muscular resistance. Reduction methods vary, with some advocating traction followed by flexion and internal rotation, while others prefer traction with gradual release combined with internal rotation and adduction. Complications such as vascular lesions or osteonecrosis of the femoral head can occur. Post-reduction management involves avoiding external rotation and early mobilization to reduce the risk of necrosis, with the timing of reduction crucial in minimizing this risk. While isolated dislocations may have lower rates of necrosis compared to associated fractures, prompt reduction remains essential for optimal outcomes.
Conclusion :
In summary, obturator hip dislocations necessitate prompt reduction, careful post-reduction management, and ongoing research to optimize outcomes and minimize complications in these rare but challenging injuries.
