Cervico-occipital hinge deformities are congenital anomalies that can have a significant negative impact on quality of life. However, these malformations may go unrecognized or unnoticed by orthopaedic surgeons. Our aim is to shed light on this pathology through this case report. 
Case report:
We report the case of a 14-year-old girl who presented to the emergency department with a closed cervical spine injury following a domestic accident. Examination in the emergency room revealed painful mobilization of the cervical spine, which remained mobilizable; palpation of the spinous processes was painless. Neurological examination revealed no sensory-motor deficit in either upper limb. X-rays of the cervical spine showed no detectable fractures or severe sprains, but did reveal a suspicious image at the C1-C2 level. A CT scan revealed a congenital malformation of the cervico-occipital hinge, with pseudarthrosis of the odontoid, itself fragmented, and a third bone fragment below the anterior arch of C1. There is also closure of the posterior arch of C2. 
Discussion and conclusion: 
Cervical spine deformities are complex problems to treat and can have a significant negative impact on quality of life by causing pain, myelopathy, radiculopathy, sensorimotor deficits, as well as the inability to maintain horizontal gaze in severe cases. An understanding of spinal biomechanics and a working knowledge of a variety of cervical radiographic parameters are essential elements in developing a surgical plan and optimizing clinical outcomes. There are many surgical options for the management of cervical spine deformities. However, choosing the right approach that ensures optimal clinical outcome can be challenging and often controversial.
