Patellar instability is an ailment that is responsible for the alteration of the mechanical functions of the knee of which the major cause is a weakness in the medial patellafemoral ligament MPFL, sometimes associated with anatomic factors predisposing to this instability.
It’s a condition that can greatly impact patient’s physical activity.
Our objective was to assess clinical and radiological results of the treatment of patellar instability by MPFL reconstruction and Elsmlie-Trillat procedure.
Methods
It was a retrospective study including 18 patients with patellar instability who underwent MPFL reconstruction and Elmslie-Trillat procedure, over a period of 3 years and 11 months (2020-23).
Result assessment of the procedure was based on clinical data (recovery of physical activity, IKDC score and radiography.
Results
The average age was 28 years with extremes of 17 and 49 years. The main symptom leading the patients to consult was knee instability, present in 100% of cases. Other symptoms were frequent as well : 71% had knee pain, 71% reported knee swelling and 86% had frequent knee giving-ways. Radiographic assessment allowed us to
notice an improvement that is however statistically unsignificant, Caton-Deschamps, Insall-Salvati and modified Insall-Salvati indexes passing respectively from a mean of 1.06, 1,35 and 1.9 before surgery to 0.93, 1,16 and 1.67 after surgery (with a p value<0.001)
Conclusion
The adequate management of patellar instability allows an improvement in symptoms and a functional recovery that enables a good retake of physical activity
